OUT PATIENT PROGRAM THERAPIST                                              

The Program Therapist interviews patients and family members to develop a complete psychosocial assessment. Provides direct-care to patients and family members through individual, family, and group therapy sessions. Serves as a member of the multidisciplinary treatment team. Participates in the planning, implementation and evaluation of treatment plans for patients. Works with patients and families to prepare aftercare plans.

Job Functions

  1. Provides direct-care services to patients and family members through interventions, family and group psychotherapy sessions. Serves as a primary therapist to assigned patients and their families.
  2. Interviews patients and family members to develop a complete psychosocial assessment. Obtains information about patient history, health and level of functioning.
  • Conducts support groups for patients / families.
  1. Serves as a member of the multidisciplinary team. Participates in the planning, implementation and evaluation of treatment plans for patients.
  2. Responds to inquiries and provides crisis intervention as needed. Acts as a resource regarding behavioral healthcare.
  3. Documents problem identification, interventions, treatment goals / plans, and patient progress/outcomes in medical record.
  • Conducts educational programs for patients and families.
  • Works with patients and families to prepare solid aftercare plans, including referrals to community agencies and other professionals.
  1. Ensures program functions within requirements established by the Joint Commission for Accreditation of Healthcare Organizations (JCAHO) and other state and federal agencies.
  2. Remains knowledgeable of current trends related to behavioral healthcare by participating in professional organizations, reviewing current literature and communicating with representatives of other facilities.
